Revocable Living Trust - Avoids Probate and Conservatorship.
Last Will and Testament - Transfers any assets outside the trust into the trust.
Durable Power of Attorney for Health Care Decisions - Authorizes someone to make healthcare decisions for you if you become incapacitated.
Declaration of Living Will - Directions about life support systems.
Directive to Physicians - Directs physicians concerning life support.
Instructions for Creator - Instructions to assist the creator of the Trust.
Memorandum to Successor Trustee - Necessary pages of Trust with checklist of what to do at death or disability.
Drafting and Recording Deed to Residence additional real estate shall be an additional charge.
