Family Services: Name Change

Name Change - Adult

Requirements:

You have lived in California for at least six weeks before filing and;
You are over 18 years of age.
You have not been convicted of a felony.
You are not changing your name to avoid creditors or to defraud somebody.
You must publish your name change in a newspaper for 3 consecutive weeks.
Name change may be granted with or without a hearing.

Name Change – Family

Requirements:

You want to change the last names (only) for several members of your family.
The members getting their name changed have not been convicted of a felony.
The members getting their name changed are not changing their names to avoid creditors or to defraud somebody.
You must publish the name in a newspaper for 3 consecutive weeks.
Name changes may be granted with or without a hearing.
If one natural parent of the minor child (ren) is not filing the petition, they may be required to sign a consent.

Includes preparing of all related documents for one adult and two children, notaries, filing.

Other Fees:

$25.00 for each additional child or adult name change.

GETTING STARTED IS EASY

To start the process for a name change simply submit your payment and complete the Name Change Questionnaire where you will submit your information (or, if mailing payment, the questionnaire page will be e-mailed to you). Our office will prepare the documents for you to sign, usually within 48 hours of receiving the completed questionnaire, and will e-mail or mail them to you.

Payment For Service
